I love The Mountain Goats, I think Heretic Pride is great, but aside from 'In the hidden places' and Wild sage' I didn't get into Get Lonely at all. The Sunset Tree on the other hand, is a fantastic album. My favourite Mountain Goats album is The Coroner's Gambit, closely followed by All Hail...

Sick at heart is an English idiom and just means to be unhappy and dispirited. I think he's evoking the feeling of depression as heartache, personally. I love this song.
